Referring to hand therapy
Who can refer?
Registered Occupational Therapists and Physiotherapists, GPs, and consultants.
What can you refer?
Any hand condition, including acquired, traumatic, neurological, rheumatological, orthopaedic, plastic and congenital conditions in adults and children. Referrals can be made from any area, locally or nationally.
How to refer?
Please email qvh.handtherapy@nhs.net
Your referral should include the patient’s personal details (including up-to-date telephone and email contact and NHS number) alongside a history of their current concern and treatment to date. If you are an external referrer, please copy the patient’s registered GP surgery into the referral.

Can I refer myself?
Adults (aged over 18 only) who are registered at Moatfield Surgery or Modality Mid Sussex (Crawley Down Health Centre, Judges Close Surgery, Park View Health Partnership, Ship Street Surgery) can self-refer to hand therapy without seeing a doctor or GP first, by completing a self-referral form. This can be emailed, posted or physically brought into the department. When you have submitted a self-referral, your referral will be triaged and placed on a waiting list, we will contact you as soon as an appointment becomes available.
Waiting times?
All referrals are triaged and seen according to patient need. Urgent referrals may be seen within 2 weeks, although routine referrals may take a few months to be seen.
